This Registration & Concent form uses Pay Pal as a  secure online payment method to accept your online registrations using a credit card for the services listed on this page. To register online for one of the packages, sessions or clinics listed here, simply click on the Register Now button next to the selection of your choice. You will then be transferred to a secure Pay Pal ordering page where you will be asked for your credit card and contact information. You will have the opportunity to print the ordering page for your records. Upon completion of the online registration process you will receive a receipt via email so make sure to imput a valid email address that you regularly use.
By registering online you also understand and agree to the folloing Concent: suzyandhondo.com, located in Bemidji, Minnesota, was organized to promote all forms of social dancing and is a private organization doing business with the state of Minnesota. By registering online I acknowledge that as a participant in any lesson or dance offered by suzyandhondo.com, I will be engaged in an activity that allows for persons with multiple levels of ability and no results are guaranteed; all fees for classes are due in advance of lessons; if I am not satisfied with the pace, content or style of teaching, I am entitled to a full refund by indicating my dissatisfaction prior to the beginning of the next class unit; and that this money back guarantee is not applicable to single, non-series type classes. I further acknowledge that suzyandhondo.com has informed me that there are risks of inuury associated with all forms of dancing. I assume such risks and agree that suzyandhondo.com, its employees, owners and affiliates will not be responsible in any way for an injury I may suffer in or as a result of participation in any classes or dances offered by suzyandhondo.com. I also understand that a review video may be produced and offered for sale upon the completion of any class, dance or class series. I hereby waive any and all claims to said production and to compensation for its use.
